If you would like to change the name on your member name tag, or if your tag was lost or broken, please fill out the form below.

Once the form is received, it may take a few weeks for the new name tag to appear in the name tag boxes; these tags are typically made in batches to save on the expensive paper they are made from. If you would like, we can contact you to let you know when your tag is ready – there is an option to indicate as such below.

Note: please do not use this form to request a name tag for your children. Currently, Beth Shalom does not supply name tags for the children of members.
