If you would like to change the name on your member name tag, or if your tag was lost or broken, please fill out the form below.

Once the form is received, please note that it may take a few months for the new name tag to appear in the name tag boxes; new tags are ordered in batches every couple of months to save costs. If you would like, we can contact you to let you know when your tag is ready – there is an option to indicate as such below.

Note: please do not use this form to request a name tag for your children. Currently, Beth Shalom does not supply name tags for the children of members.
